Mc's Snackbox, a takeaway at 52 Main Street, Garvaghy, Portglenone, Antrim was given the score after assessment on September 20, the Food Standards Agency's website shows.
It means that of Mid and East Antrim's 125 takeaways with ratings, 90 (72%) have ratings of five and none have zero ratings.
